  • Return to Play/Participation Letter and Form

    In consultation with our medical director and based on the updated guidance from the American Academy of Pediatrics, we strongly encourage all parents to utilize the Return to Play/Participate form prior to returning to physical activity (i.e., physical education or athletics). This is in addition to the already required note from a healthcare provider stating a child who has had COVID-19 is cleared to return to school.

  • COVID-19 Student/Staff Return to School Form

    The District will follow CDC and NYSDOH guidance for allowing a student or staff member to return to school/work after exhibiting signs of COVID-19. Parents/Staff will be required to seek medical attention and MUST have a note from a healthcare provider (physician, nurse practitioner, or physician assistant) indicating they can return to school/work.

  • COVID-19 Testing and Contact Tracing Plan

    In compliance with CDC and NYSDOH recommendations, the district will not conduct COVID-19 testing nor require COVID-19 testing or antibody testing of students or staff. The decision of whether a test needs to be conducted should be determined by a healthcare provider or the Erie County Department of Health.
